EVSC Health and Wellness Centers
The EVSC Health and Wellness Centers are exclusively for employees and their dependents who are covered under an EVSC employee insurance plan. A comprehensive overview of the centers is available on the EVSC website by clicking here.

The locations of the clinics are:
  • Lincoln Avenue Center (1332 Lincoln Ave.) – located two blocks east of Highway 41 in the Family Medical Associates building at the corner of Lincoln Avenue and S. Willow Road.
  • Mt. Pleasant Center (1033 Mt. Pleasant Rd., Suite 1) – located just east of Highway 41 in the 41 North Business Park, off Mt. Pleasant Road, behind McDonald’s.
  • Tri-State Community Clinic (5625 Pearl Dr., Suite D) - located by the UPS Store.

Eligible employees may make an appointment online or by calling 888-492-8722. Hours of the centers are:
  • Monday: 7 a.m. – 5 p.m.
  • Tuesday: 9 a.m. – 7 p.m.
  • Wednesday: 11 a.m. – 5 p.m.
  • Thursday: 7 a.m. – 5 p.m.
  • Friday: 7 a.m. – 4 p.m.

Staffed by experienced primary care physicians and nurse practitioners, the Centers offer identical services. Services to be provided include:
  • Primary care:  management of chronic conditions and acute care needs
  • Annual physicals, sports physicals, well visits, and immunizations (seasonal flu, hepatitis B, tetanus boosters only)
  • Minor illnesses and injuries (sore throat, flu, seasonal allergies, sprains and strains)
  • Screenings: high blood pressure, blood sugar, influenza, strep throat
  • Pharmacy and lab services
  • Health coaching
  • 24-hour Nurse Call Line
