In order to be eligible to participate in the Service Learning program and/or be eligible for a classroom grant of up to $500, teachers must submit this form no later than 4:00 PM on September 12, 2014. Data may not be saved into this form online. It is recommended that applicants prepare responses separately and copy and paste them into this form.
